AFC Member Hurricane Michael Relief Application

 Criteria to receive funds:

• Must be an AFC Member in good standing and in one of these categories: Regular, Retiree, Life, Adjunct/Part-time Employee or Honorary Life member classification)
• Please submit a photo (if possible) of the nature of the loss
• Funds may be requested for food, clothing, transportation, hotels or shelter, repairs of home or other, childcare or other health related concerns.
